AP neglected AV as defined in OAR 4110200002 (1)(b)(A)(i) failure to care plan aggressive behaviors of W5, creating risk of serious harm to AV and other vulnerable residents. (ii) failure to safety plan behaviors has resulted in multiple resident to resident altercations most recently causing injury to AV by physical harm, causing unreasonable discomfort to AV.
