Findings
AP1 neglected AV as defined in OAR 4110200002 (1) (b)(A)(i) by failing to provide supervision for safety defined as failure to train and oversee AP2 and AP3, resulting in AP2 and AP3 failing to react according to policy when AV fell. This failure resulted in AV enduring pain for several days before being sent to the hospital.
