AP neglected AV as defined in OAR 4110200002 (1)(b)(A)(i) AV suffered a fall on 5/5/19 resulting in a fracture to the right humerus, creating pain and discomfort to AV. AV then suffered a second fall on 5/6/19 resulting in complaint of head and neck pain creating risk of serious harm to AV as well as significant pain and discomfort.
