APS is assigned due to AP neglecting AV as defined in OAR 4110200002(1)(b)(A)(i)(ii) by failing to provide AV with the basic care and services needed to keep AV safe from risk of harm and loss of personal dignity by leaving AV sitting in h/h clothes/wheelchair that were saturated in urine, in the community with others, for over two hours.
