APS is assigned due to AP neglecting AV as defined in OAR 4110200002 (1)(b)(A)(i)(ii) by failing to provide the basic care, services, and supervision needed to keep AV safe from harm and injury resulting in AV falling out of bed and due to failure to properly place AV's fall mat next to h/h bed, AV obtained a 1 cm abrasion on the left side of h/h forehead.
