Findings
The facility failed to provide service and appropriately care plan for Alleged Victim (AV) regarding his/her history of falls and risk of falls. On or about May 17, 2019, AV was found on the floor from a fall, was transported to the hospital for treatment and was diagnosed with a wrist fracture. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
