Findings
AP1 neglected AV as defined in OAR 4110200002 (1) (b)(A)(ii) by failing to provide basic care to maintain the health and safety of the AV. AV was left in soiled incontinence brief for seven hours and AV was not repositioned for several hours creating a risk of serious harm.
