Findings
On or about May 18, 2019, Alleged Victim (AV) was found to have an unexplained bruise on his/her lower back. An investigation determined that AV more than likely sustained the bruising during a transfer. The facility failed to provide AV safe assistance with each transfer, which resulted in significant bruising to AV's lower, right back. The facility's failure to properly plan care for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
