Findings
Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan by failing to keep eyes on AV while assisting him/her in the bathroom. AP2 left AV in the bathroom and assisted another staff with another resident. AV tried to get up from the toilet and fell. AP2's actions are considered neglect of care, which is a violation of resident rights and constitutes abuse. The facility failed to provide a safe environment, which is a violation or Oregon Administrative Rules.
