- Alleged Victim (AV) is a known fall risk and is care planned to be checked on eight times per shift, which is every hour, due to repeated falls when AV is trying to self-transfer while using the restroom. On or about June 20, 2019, at approximately 6:30am, AV fell when attempting to self transfer from the toilet to his/her wheelchair causing AV bruising. Prior to the fall AV was checked on around 5:00am. The facility failed to follow AV's care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
