Findings
Witness 1 (W1) and Alleged Victim (AV) had a known history of conflict. On or about June 26, 2019 W1 got into a conflict with AV causing AV emotional distress. The facility failed to care plan appropriately, which is a violation of resident rights, is neglect of care and constitutes abuse.
