Findings
Alleged Victim (AV) had a known history of turning off his/her tab alarm. On or about July 11, 2019, AV fell causing AV pain to his/her arm and back. AV's tab alarm was not on at the time of the fall. The facility failed to care plan according to AV's history of turning off his/her tab alarm which is a violation of resident rights, is neglect of care and constitutes abuse.
