Witness 1 (W1) had a known history of aggressive behavior. On or about July 15, 2019, Alleged Victim (AV) was showing W1 to his/her room and W1 hit AV. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
