Findings: The facility failed to properly document the Alleged Victim's medical records to be able to treat the resident as needed for constipation. The facility failed to have a consistent method of ensuring when AV was or was not having bowel movements, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
