Findings
Alleged Victim (AV) is a fall risk and is care planned for a pool noodle to be used while in bed as a barrier. On or about July 25, 2019, staff responded to AV's room after hearing a loud noise. AV was found lying on the floor with skin tears. The pool noodle was not in place at the time of the fall. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
