Findings
The facility failed to provide a safe environment for the Alleged Victim (AV). Sometime between the evening of September 3, 2019 and the morning of September 4, 2019, AV successfully eloped from the facility through an open gate, exposing him/her to harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
