Findings
The facility failed to ensure supervision, appropriate interventions and staff support regarding risks of resident to resident altercations. Three separate incidents occurred between the Alleged Victim and Witness #1 where Witness #1 struck AV unprovoked. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
