Findings
On or about August 7, 2019, the Alleged Victim (AV) fell from his/her wheelchair while propelling around the facility and received a laceration to his/her forehead. The facility failed to follow AV's care plan to be alert when AV is propelling around the facility as AV is known to try to stand using hand rails. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
