Findings
On or about September 13, 2019, Alleged Victim (AV) was wondering in a hallway without his/her walker. Alleged Perpetrator 2 (AP2) placed a chair behind AV and put his/her hands on AV's shoulder and pushed AV down onto the chair, causing AV to become upset and aggressive with AP2. The facility had previous conversations with AP2 regarding prior incidences with other residents. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide AV with a safe environment which is a violation of Oregon Administrative Rules.
