Findings
On or about September 17, 2019, Alleged Victim (AV) was found with bruising to his/her wrist, knee and was in pain. It was unclear when and how the bruising occurred, and no indication that any medical evaluation of the injury had been completed. On or about September 10, 2019 and September 14, 2019 AV experienced falls causing the bruising and increased pain. The facility failed to assess and intervene to AV's change of condition, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
