The facility failed to follow Witness 1's (W1) care plan to redirect him/her away from the Alleged Victim's (AV) room to prevent altercations. The failure resulted in AV being kicked by W1 causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
