Findings
The facility failed to provide a safe environment for the Alleged Victim or to properly plan care surrounding falls and injuries of unknown cause. The Alleged Victim had multiple unwitnessed falls and injuries, one which resulted in AV receiving stitches to his/her face. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
