- Because the facility closed prior to the completion of this investigation report, no Civil Penalty can be levied against this owner for this action. The facility failed to provide a safe medication administration system for Alleged Victim (AV) by not ensuring his/her medication was onsite and administered as ordered. From approximately August 30, 2019 through September 17, 2019, AV did not receive his/her mental health medication and/or his/her behavior medication on multiple occasions. AV began to experience mental health symptoms and was sent to the hospital sometime in September 2019, but the exact date is unclear due to a lack of facility documentation. AV was discharged from the hospital on September 18, 2019 but continued to experience mental health symptoms. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
