- Because the facility closed prior to the completion of this investigation report, no Civil Penalty can be levied against this owner for this action. The facility failed to provide a safe medication administration system for Alleged Victim (AV) by not ensuring his/her blood pressure medications were onsite and administered as ordered. Facility documentation indicated that from approximately August 3, 2019 through October 4, 2019, AV did not receive his/her blood medications as ordered and from approximately August 26, 2019 through October 4, 2019, AV’s medical order to have his/her pulse be taken prior to medication administration was not recorded. AV was sent to the hospital for low blood pressure but due to lack of facility documentation, the exact date is unclear. The facility's failures are a violation of resident rights, are considered neglect of care and constitutes abuse.
