- Because the facility closed prior to the completion of this investigation report, no Civil Penalty can be levied against this owner for this action. The facility failed to provide a safe medication administration system for Alleged Victim (AV) by not ensuring his/her mental health medication was onsite and administered as ordered. From approximately September 14, 2019 through September 18, 2019, AV did not receive his/her mental health medication due to the facility not ordering prior to running out, resulting in AV having increased mental health symptoms. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
