Findings
On or about September 21, 2019, Alleged Perpetrator 2 (AP2) failed to follow the care plan for the Alleged Victim (AV) when AP2 left AV unattended, sitting on the edge of his/her bed while AP2 was getting a brief. AV stood up and fell, causing a skin tear. AP2's actions are considered neglect of care which constitute abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
