- Alleged Victim (AV) frequently leaves the facility with medications needed as packaged by facility staff. On or about August 23, 2019, AV left the facility with medications packaged by facility staff. Upon reviewing the medication, while out of the facility, two pills looked different from what AV normally takes. After taking his/her medication AV became ill with vomiting and diarrhea. The facility failed to provide a safe medication administration system which is a violation of resident rights, is considered neglect of care and constitutes abuse.
