Findings
Alleged Victim (AV) had known exit seeking behaviors and was care planned to have safety checks regularly around the clock. On or about September 30, 2019, at approximately 8:00pm, Alleged Perpetrator 2 (AP2) was notified by Witness 4 (W4) that AV had an earlier elopement attempt. AP2 attempted to check on AV but AV’s door was locked and AP2 failed to conduct a visual check of AV. AV successfully eloped from the facility and sustained an injury. AP2’s action is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
