Findings
Alleged Victim (AV) had a known history of exit seeking. On or about September 5, 2019, AV exited the facility through an unlocked door and was found by law enforcement approximately 12 blocks away. The facility failed to provide a safe environment placing AV at risk for harm which is a violation of resident rights, is considered neglect of care and constitutes abuse.
