Findings
The facility failed to provide appropriate supervision and monitoring of the Alleged Victim (AV) according to a history of being restrained to his/her wheelchair. The failure resulted in approximately 6 separate occasions of AV being restrained to his/her wheelchair causing risk of harm and loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
