Findings
Alleged Victim (AV) had a known history of falls out of bed. On or about September 29, 2019, AV had injuries to his/her right side of body. It is presumed that AV fell out of bad. The facility failed to care plan and implement interventions to mitigate AV's risk of falling out of his/her bed which is a violation of resident rights, is neglect of care and constitutes abuse.
