Findings
On or about October 3, 2019, Alleged Perpetrator #2 (AP2) administered the incorrect medication to the Alleged Victim (AV), causing him/her to be transferred to the hospital for treatment. AP2's actions are considered neglect of care and constitute abuse. The facility failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
