Findings
Alleged Victim (AV) was readmitted to the facility on or about October 1, 2019 with changed medication orders. On or about October 5, 2019, the facility discovered an error and AV did not receive his/her medications as ordered. AV did not experience any negative symptoms due to not receiving his/her medications as order. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
