Findings
The facility failed to provide a safe medication administration system and ensure the Alleged Victim's medication was administered as ordered. The failure resulted in AV being administered his/her medication incorrectly for over 1 week and exposed him/her to risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
