- Witness 1 (W1) was receiving one on one care due to agitation and a resident to resident incident that occurred earlier in the morning. Staff providing one on one care had to leave to go to a meeting. W1 approached Alleged Victim (AV) in the dining room and assisted him/her to a standing position. AV stated something about going outside and started screaming. W1 grabbed AV's hair with both hands and began pulling causing AV unreasonable discomfort. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
