Findings
The facility failed to appropriately care plan and implement interventions regarding Alleged Victim's (AV) known exit seeking behaviors. On or about October 14, 2019, AV exited the facility and sustained minor scrapes to his/her legs. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
