Findings: The facility failed to follow the Alleged Victim’s care plan to provide a two-person transfer while moving AV. Staff tried to transfer AV alone on two occasions on October 15, 2019, the first resulting in AV being lowered to the ground without injury, the second fall resulted in AV hitting his/her head on the wall. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00254 $375.00 fine assessed
