Findings
The facility failed to provide a safe environment by ensuring supervision and staff support regarding known behaviors between the Alleged Victim (AV) and Witness #5 (W5). Many incidents have occurred between the two where W5 hit AV, however, interventions were not successful in keeping AV safe from w5. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
