Findings
Alleged Victim (AV) and Witness 1 (W1) had a prior altercation. On or about October 20, 2019, evidence suggest an un-witnessed altercation occurred between AV and W1 causing AV injury. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
