Findings
The facility failed to ensure the Alleged Victim (AV) had a safe environment. On or about October 22, 2019, it was discovered that the AV had scratches on his/her back. It was determined that the scratches could have been coming from a counter in the AV’s bathroom. The facility failed to pad the counter in the bathroom, causing an unsafe environment for the AV. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
