Findings
On or about October 23, 2019, Alleged Victim (AV) did not receive his/her evening medications due to an error with the MAR system. On or about October 24, 2019, AV had his/her blood drawn and his/her INR was below optimal range. The facility failed to provide a safe medication administration system putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
