- Alleged Victim (AV) relies on facility staff for assistance. On or about October 15, 2019, AV asked Alleged Perpetrator 4 (AP4) to assist his/her to the toilet. AP4 did not assist AV to the toilet and instead transferred his/her to his/her recliner, leading to an incident of fecal incontinence. On or about October 21, 2019, AP4 was disciplined by the facility in writing for his/her refusal to assist AV to the toilet. AP4's refusal to assist AV is considered neglect of care and constitutes abuse. The facility failed to protect AV from AP4's actions which is a violation of resident rights, is neglect of care and constitutes abuse.
