Findings
On or about October 30, 2019, the Alleged Victim (AV) experienced a non injury fall and attempted to use the facility call system for assistance. The facility failed to have a functioning call system causing AV to go approximately two hours without assistance, which is a violation of Oregon Administrative Rules.
