Findings
The facility failed to appropriately care plan related to the Alleged Victim's (AV) change of condition. The failure resulted in the facility care staff not knowing AV required 2 person assistance with transfers, causing AV to fall and experiencing a fracture to his/her leg, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
