Findings
The facility failed to follow the Alleged Victim's (AV) care plan to ensure his/her tab alarm was functioning properly and a pressure alarm was in place. The failure resulted in AV experiencing a fall causing unreasonable discomfort and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
