Findings
On or about November 8, 2019, Alleged Perpetrator 2 (AP2) failed to give Alleged Victim (AV) medication as ordered. An investigation determined that AP2 gave AV hospice medication prescribed to another resident. AV was transported and admitted to the hospital for accidental overdose. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
