Findings
The facility failed to ensure supervision and staff support regarding known behaviors and fall risks related to the Alleged Victim and Witness #1. An incident occurred between the two where Witness#1 grabbed the Alleged Victim by the arm and shook him/her. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
