Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1. An incident occurred between the Alleged Victim and Witness #1 where Witness #1 grabbed the Alleged Victim by the shoulder and squeezed very hard, causing unreasonable discomfort to the Alleged Victim. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
