Findings
On or about November 17, 2019, Alleged Perpetrator #2 (AP2) gave the Alleged Victim (AV) incorrect medication, placing AV at a risk for harm and causing AV to be transported to the hospital for observation. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
