Findings
The facility failed to follow the Alleged Victim's (AV) care plan to perform hourly checks due to his/her known fall history. The failure resulted in AV experiencing an unwitnessed fall causing unreasonable discomfort and went approximately four hours without being checked on, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
